Hello and welcome to our Student Registration page!

Please see the categories below to aid in student registration, apply for open enrollment, or update general forms.

**If applying for new open enrollment, please DO NOT start a Final Forms account until after your student has been approved.**

Back to School Forms-Current Students

All back to school forms will be completed in Final Forms.

Final Forms can be completed from any mobile device, including your phone. This program also allows you to create your own username and password. For assistance, you can refer to this help document:  Parent Playbook.

If you are editing a current student's email in Final Forms, please note that a school email address cannot be used. You must use a personal email account such as @yahoo.com or @gmail.com.

Returning open enrollment forms must be filled out no later than February 28th. Scroll down to the end of this page to access the returning open enrollment form.

Kindergarten Enrollment

Kindergarten Enrollment: Parents living in the Firelands school district with children who will be five years old by August 1, 2025 are eligible to register their child for 2025-2026 kindergarten starting on March 1, 2025. Online registration can even be completed on your phone, without downloading an app.

Open enrollment for parents living out of the district opens on February 3, 2025 for the 2025-2026 school year. For new open enrollment applications, registration is not permitted until your application has been accepted and the registration window opens. (Please see the open enrollment section below).

Kindergarten screening is a separate event that will take place in May 2025, but your time slot must be set up during the registration process in March.

Documents required for registration include your child's birth certificate, current proofs of residency, parent/guardian photo ID, shot records, and custody papers (if they apply). 

Residency documents must have name, current address and current date visible or will not be accepted. You will need the following documents, one from each list below:

Choose 1:  current signed rental agreement, current home mortgage statement, purchase agreement, or construction contract
Choose 1:  current tax bill, current utility invoice (gas, water or electric), homeowner/renter insurance statement, or current paystub with new address

If you are living in a Firelands Local School District home that you do not own or rent, a residency affidavit must be completed as part of proving residency. That form is located here. 

Kindergarten registration involves completing all of the online forms in the Final Forms portal and providing all requested documents. The Final Forms link is here. (Open enrollment applications must be submitted and approved prior to registration).

New Student Online Registration (1-12)

**The application for open enrollment is in the next section. Final Forms is for registration after applications have been accepted.**

Online registration (through Final Forms) is open for students who are moving into the Firelands School District during the school year. Please make sure you are selecting the correct year.

Registration involves completing all of the online forms in the Final Forms portal and providing the listed documents below. The Final Forms link is here.

The following documents can be uploaded directly into your online account or emailed to [email protected]:

  1. Child's birth certificate
  2. Immunization records (we accept printouts from MyChart or similar accounts)
  3. Parent or guardian photo ID
  4. Two proofs of residency, one from each list. Documents must show name, current address, and current date or they will not be accepted.
  • Choose 1: current signed rental agreement, current home mortgage statement, purchase agreement or construction contract
  • Choose 1: current tax bill, current utility invoice (gas, electric or water), home or rental insurance statement, or current paystub with new address

5. Special education or gifted documents, if applicable (IEP, ETR, 504, WEP, etc.)
6. Court papers including custody papers if applicable

Welcome to Firelands!

Open Enrollment

Open Enrollment for 2024-2025 is closed.

NEW Open Enrollment for 2025-2026: We have availability for open enrollment as follows. The form will open on February 3, 2025 and can be found at this link: https://forms.gle/Y8sQVfVSMavuVPha6

  • Kindergarten - not taking applications, availability to be determined by June 15th

  • Grade 1 -  no openings

  • Grades 2, 3, and 4 - will consider siblings for existing families only, depending on space available

  • Grade 5 - space available for 5 students

  • Grade 6, 7, and 8 - no openings

  • Grade 9 - 10 spots for current 8th graders

  • Grades 10, 11, and 12 - no openings

RETURNING Open Enrollment: If your child was open-enrolled this year, you must confirm that they are continuing in 2025-2026 by filling out the quick online form indicated below when it opens on 2/3/2025. You will receive confirmation of your child's continued open enrollment status by e-mail from the Superintendent's Office. The deadline for this form is February 28, 2025. Failure to return the form would indicate no plans to return for 2025-2026. The application is available at this link: https://forms.gle/reKnsdLcATVwXVom7.

Open Enrollment Due to Moving Out of District  (Link: https://forms.gle/vSJFznmiJKoUyxWp6)
If an already enrolled Firelands District family moves out of the district, they will need to apply for open enrollment status and be approved to keep their students in Firelands schools.  The “Moving Out of District” open enrollment form must be completed, and proof of residency documents must be provided. If approved by the Superintendent, students will become open enrolled.
 
If not approved, the students must be withdrawn from Firelands and enroll in their new school district within 10 days of notification from the Superintendent's Office. If a family moves out of district and fails to apply for open enrollment, the students must be withdrawn from Firelands and enroll in their new school district within 10 days of notification from the Superintendent's Office.
