Residency Form

Sworn Statement of Residency

If you are living with a resident or renting within the Firelands Local School District, this form must be completed as part of proving residency.

Registration & Change of Information

Withdrawal Form

This form is required for all withdrawals. Please contact the main office at your student's school for more information.

Change of Address Form

Changes to PowerSchool will be made after you submit this form and supporting documents. You must also enter the address change in Final Forms.

However, if you are moving out of district, do not use the Change of Address Form. You must be approved for open enrollment after completing the Moving out of District Open Enrollment application. The application must be approved by the Superintendent. You also need to make the address change in Final Forms.

If a family moves out of district and fails to apply for open enrollment, the students must be withdrawn from Firelands and enroll in their new school district within 10 days of notification from the superintendent.
